the only time i see that happen is with nexerelin installed and have dispatched base strike fleet against a pirate/pather base.
typically they mop up all the remaining hostile forces and take over their installations after the fight, so it's entirely possible for the player to visit it afterwards and discover friendly installations.
apart from that, i dunno.

is there a way to remove stellar shades from a planet? it's conflicting with a mod's function but i didn't realize it at the time until it was too late

used a terraform mod to remove heat from a planet but it ended up conflicting with industrial evolution's rift generator because stellar shades prevent the device from working anymore
