Do the effects of the surveying equipment mod stack?  I know that the min cost is 5 supplies, but does having multiple surveying ships lower the cost over just one for big planets?

Surveying equipment does scale, but it's surveying cost reduction cannot go below the minimum value.
Salvage rig bonus scales as well, but with diminishing returns. All ships but the first one have their bonus reduced more and more. It's also automatically done in such a way that you get the biggest possible bonus, so you don't have to worry about order of your ships messing this up.

I'll like to add that if you have one ship with salvage gantry, the refit screen will tell you how much the next salvage gantry from the same ship hull will give you. So you aren't completely in the dark.
